Text messaging support for people with chronic disease

What will patients receive?

  • Program participants will receive customised text messages over 12 months.
  • Participants will be sent 4 text messages per week for up to 12 months
  • Text messages will be sent at different times during the working day
  • Text messages will be individualised based on the patients disease condition & will provide advice, motivation, information & support. Including:
  • – Advice about monitoring of risk factors
  • – Managing exacerbations
  • – Tips to engage in healthy behaviours
  • – Participants will also receive useful links to other services such as Get Healthy and Quit line.

Text message topics include:

  • Healthy eating
  • Physical activity
  • Diabetes and cardiovascular disease self-management
  • Medication
  • Insulin
  • Smoking cessation

Who is eligible?

To be eligible for SupportMe, participants must:

  • Be over 18-years of age and can provide informed consent
  • Have either:
    Coronary heart disease (CHD)
    AND/OR
    Type 2 Diabetes (with an HbA1C of 7.1% to 11.4%)
  • Own a mobile phone
  • Can read text messages in English

Text Message Development

The text messages were created and tested by a working group including clinicians (medical, nursing, allied health) involved in the care of people with chronic disease, primary care, community health, population health, existing health promotion programs and consumers.
